Ensure that support services and interventions are person centered/strength based/recovery oriented</li> <li>Collaboratively develop and implement viable pathways to short/long term sustainable housing with people served.</li> <li>Forge and maintain positive and productive working relationships with community-based stakeholders and coalitions&nbsp;&nbsp;</li> <li>Provide ongoing assessment of client needs for clinical, supportive services, and safety issues. &nbsp; Develop, modify and implement individual client service plans to address ongoing clinical and/or systemic needs – including risk factors.&nbsp;&nbsp;</li> <li>&nbsp;Recognize and respond in real time to sudden changes in client behavior, physical health and/or related medical issues, or other related areas of concern.&nbsp;&nbsp;</li> <li>Develop and implement crisis response planning as needed.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</li> </ul> </td> </tr> <tr> <td> <p><strong>Qualifications:</strong></p> <ul> <li>Requires a Bachelor’s degree in a human services field; social work, psychology, human services, or sociology preferred or 2 years case management or comparable experience with the homeless population.</li> <li>A Master’s degree in the healing arts is preferred.</li> <li>Must possess a valid Massachusetts driver’s license and maintain a personally owned vehicle</li> </ul> <p><strong>Compensation:</strong></p> <ul> <li>Bachelors: $48,000.00 annually</li> <li>Masters: $50,000.00 annually</li> </ul> </td> </tr> <tr> <td><strong>Schedule:</strong> Monday-Friday, 9am-5pm</td> </tr> </tbody> </table><div class="content-pay-transparency"><div class="pay-input"><div class="title">Hourly Rate</div><div class="pay-range"><span>$48,000</span><span class="divider">&mdash;</span><span>$50,000 USD</span></div></div></div><div class="content-conclusion"><p>At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
